The Developmental Disabilities Health Services, PA (DDHS), a specialized health care and care management provider for persons with developmental disabilities, has been awarded a $3.7 million Health Care Innovation Challenge grant by the Centers for Medicare and Medicaid Services’ Center for Medicare and Medicaid Innovation. DDHS employs a Developmental Disabilities Health Home (DD Health Home) model of health care delivery for persons with developmental disabilities that integrates primary care, mental health services and specialty medical care which addresses the three aims of the Health Care Innovation Challenge and health care reform, improving health and the individual experience of care, and reducing the per capita costs of care for this high-risk population. With the awarded funding DDHS will confirm and enhance preliminary findings related to improvements in patient health, verify preliminary findings related to reduced acute care costs, expand the model in New Jersey, replicate the model in New York and Arkansas, investigate the effects of the model on long-term care quality, utilization and cost, and enhance workforce development for nurse practitioners and physicians who may choose to work in this model.
